To be eligible for the Thrive Support Program, applicants must meet the following criteria:
Have a cancer diagnosis, to be confirmed with an oncology report direct from hospital or cancer agency;
Be a Canadian citizen or permanent resident and living in Canada;
Qualify as low-income according to Statistics Canada and provide proof with income tax notice of assessment;
Provide reference and consent of legal guardian if 16 years of age or younger. In this case, the legal guardian(s) may be required to submit supportive documents;
For Thrive Support Program applicants, you already know the service, treatment and/or health practitioner in Canada of which you are requesting funding support;
Complete all steps of the required application and submit supportive documents.
